The hospital was never enough: Healthcare as urban design

The core message of the session is that hospitals cannot be the starting point for health: cities, homes, streets, food systems, environmental quality, and everyday design choices shape health long before a patient reaches clinical care. The moderator framed the discussion as a shift from “health care system delivered” toward health designed into daily life, especially for a 2050 city where longer lives should also mean healthier years. The panel was positioned across three complementary perspectives: public health, clinical care, and the built environment. Doctor Rashed Al Suwaidi explained that the session title was not provocative for Abu Dhabi Public Health Centre; it was the centre’s founding idea. He described Abu Dhabi’s public health mandate as targeting the determinants of health: what people eat, the air they breathe, the actions they take, and the way they live. His emphasis was prevention over treatment, with the public health system working to keep people away from hospitals by making healthier choices easier, more visible, and more natural. Examples included NutriMark in supermarkets to help consumers understand food choices, health-awareness efforts, guideline and label programs, school screening for students and parents, and a wellness program for adults that informs people about their health and genome. Doctor Georges-Pascal Haber then connected this prevention agenda to what clinicians see inside hospitals. He argued that many hospital patients arrive with diseases that began years earlier. Using kidney cancer as an example, he described a progression from seeing patients with cancers that had already spread, to treating advanced but localized cancers, to detecting smaller early-stage tumors through screening and removing only the tumor while preserving the kidney. He extended this logic to diabetes-related blindness, stroke linked to hypertension, premarital screening, genetic mutation detection at the embryo level, and the possibility of future correction of mutations before birth. His conclusion was that early detection, screening, prevention, and healthier environments can reduce disease burden and the need for hospitals. Sandra Woodall reframed the theme from “healthcare as urban design” to “urban design as healthcare.” Her “hospital without walls” concept means extending specialist care, diagnostics, and health pathways beyond formal facilities into the places where people live every day: homes, streets, and neighborhoods. She argued that these environments can help people stay well, support recovery, and maintain independence. Drawing on her experience with Tangram, healthcare design, and the Al Ain city master plan, she presented the oasis not simply as landscape, but as life-sustaining infrastructure that moderated the environment, enabled food to grow, nourished communities, and promoted health and well-being in a harsh desert setting. The provided transcript ends as she begins discussing the 1980s master plan for Al Ain City.
